Identify the exact location
Start with the community name, street address and license number supplied by the operator. A brand may have several locations, and a marketing name may differ from the licensed name.
Find the official record
Open Arizona’s licensing resources or AZ Care Check. Match the address and license number, rather than relying on a similar name. If the identity is unclear, ask the operator to clarify before using the result.
Read the record in context
Review the status, dates, facility type and authorized services where available. Licensed capacity is the maximum allowed, not a count of open rooms. A source-record date does not mean GlenOak visited or inspected the location on that date.
Read available inspection information
Keep inspection and enforcement information in its original context, including dates and published correction information. A license does not guarantee quality or suitability. Missing information in a directory is not proof of a clean inspection history.
Resolve the remaining questions
Ask the community how its current services match the person’s needs. Request an itemized quote and confirm availability directly. Keep the exact facility identity and the date you checked with your notes.
